Objective:
White matter hyperintensity (WMH) burden is greater, has a frontal-temporal distribution, and is associated with proxies of exposure to repetitive head impacts (RHI) in former American football players. These findings suggest that in the context of RHI, WMH might have unique etiologies that extend beyond those of vascular risk factors and normal aging processes. The objective of this study was to evaluate the correlates of WMH in former elite American football players. We examined markers of amyloid, tau, neurodegeneration, inflammation, axonal injury, and vascular health and their relationships to WMH. A group of age-matched asymptomatic men without a history of RHI was included to determine the specificity of the relationships observed in the former football players.
Participants and Methods:240 male participants aged 45-74 (60 unexposed asymptomatic men, 60 male former college football players, 120 male former professional football players) underwent semi-structured clinical interviews, magnetic resonance imaging (structural T1, T2 FLAIR, and diffusion tensor imaging), and lumbar puncture to collect cerebrospinal fluid (CSF) biomarkers as part of the DIAGNOSE CTE Research Project. Total WMH lesion volumes (TLV) were estimated using the Lesion Prediction Algorithm from the Lesion Segmentation Toolbox. Structural equation modeling, using Full-Information Maximum Likelihood (FIML) to account for missing values, examined the associations between log-TLV and the following variables: total cortical thickness, whole-brain average fractional anisotropy (FA), CSF amyloid ß42, CSF p-tau181, CSF sTREM2 (a marker of microglial activation), CSF neurofilament light (NfL), and the modified Framingham stroke risk profile (rFSRP). Covariates included age, race, education, APOE z4 carrier status, and evaluation site. Bootstrapped 95% confidence intervals assessed statistical significance. Models were performed separately for football players (college and professional players pooled; n=180) and the unexposed men (n=60). Due to differences in sample size, estimates were compared and were considered different if the percent change in the estimates exceeded 10%.
Results:In the former football players (mean age=57.2, 34% Black, 29% APOE e4 carrier), reduced cortical thickness (B=-0.25, 95% CI [0.45, -0.08]), lower average FA (B=-0.27, 95% CI [-0.41, -.12]), higher p-tau181 (B=0.17, 95% CI [0.02, 0.43]), and higher rFSRP score (B=0.27, 95% CI [0.08, 0.42]) were associated with greater log-TLV. Compared to the unexposed men, substantial differences in estimates were observed for rFSRP (Bcontrol=0.02, Bfootball=0.27, 994% difference), average FA (Bcontrol=-0.03, Bfootball=-0.27, 802% difference), and p-tau181 (Bcontrol=-0.31, Bfootball=0.17, -155% difference). In the former football players, rFSRP showed a stronger positive association and average FA showed a stronger negative association with WMH compared to unexposed men. The effect of WMH on cortical thickness was similar between the two groups (Bcontrol=-0.27, Bfootball=-0.25, 7% difference).
Conclusions:These results suggest that the risk factor and biological correlates of WMH differ between former American football players and asymptomatic individuals unexposed to RHI. In addition to vascular risk factors, white matter integrity on DTI showed a stronger relationship with WMH burden in the former football players. FLAIR WMH serves as a promising measure to further investigate the late multifactorial pathologies of RHI.

Background
Imbalance and gait disturbances are common in patients with vestibular schwannoma (VS) and can result in significant morbidity. Current methods for quantitative gait analysis are cumbersome and difficult to implement. Here, we use custom-engineered instrumented insoles to evaluate the gait of patients diagnosed with VS.
MethodsTwenty patients with VS were recruited from otology, neurosurgery, and radiation oncology clinics at a tertiary referral center. Functional gait assessment (FGA), 2-minute walk test (2MWT), and uneven surface walk test (USWT) were performed. Custom-engineered instrumented insoles, equipped with an 8-cell force sensitive resistor (FSR) and a 9-degree-of-freedom inertial measurement unit (IMU), were used to collect stride-by-stride spatiotemporal gait parameters, from which mean values and coefficients of variation (CV) were determined for each patient.
ResultsFGA scores were significantly correlated with gait metrics obtained from the 2MWT and USWT, including stride length, stride velocity, normalized stride length, normalized stride velocity, stride length CV, and stride velocity CV. Tumor diameter was negatively associated with stride time and swing time on the 2MWT; no such association existed between tumor diameter and FGA or DHI.
ConclusionsInstrumented insoles may unveil associations between VS tumor size and gait dysfunction that cannot be captured by standardized clinical assessments and self-reported questionnaires.

This article is a clinical guide which discusses the “state-of-the-art” usage of the classic monoamine oxidase inhibitor (MAOI) antidepressants (phenelzine, tranylcypromine, and isocarboxazid) in modern psychiatric practice. The guide is for all clinicians, including those who may not be experienced MAOI prescribers. It discusses indications, drug-drug interactions, side-effect management, and the safety of various augmentation strategies. There is a clear and broad consensus (more than 70 international expert endorsers), based on 6 decades of experience, for the recommendations herein exposited. They are based on empirical evidence and expert opinion—this guide is presented as a new specialist-consensus standard. The guide provides practical clinical advice, and is the basis for the rational use of these drugs, particularly because it improves and updates knowledge, and corrects the various misconceptions that have hitherto been prominent in the literature, partly due to insufficient knowledge of pharmacology. The guide suggests that MAOIs should always be considered in cases of treatment-resistant depression (including those melancholic in nature), and prior to electroconvulsive therapy—while taking into account of patient preference. In selected cases, they may be considered earlier in the treatment algorithm than has previously been customary, and should not be regarded as drugs of last resort; they may prove decisively effective when many other treatments have failed. The guide clarifies key points on the concomitant use of incorrectly proscribed drugs such as methylphenidate and some tricyclic antidepressants. It also illustrates the straightforward “bridging” methods that may be used to transition simply and safely from other antidepressants to MAOIs.

Background
Alcohol use disorder (AUD) and schizophrenia (SCZ) frequently co-occur, and large-scale genome-wide association studies (GWAS) have identified significant genetic correlations between these disorders.
MethodsWe used the largest published GWAS for AUD (total cases = 77 822) and SCZ (total cases = 46 827) to identify genetic variants that influence both disorders (with either the same or opposite direction of effect) and those that are disorder specific.
ResultsWe identified 55 independent genome-wide significant single nucleotide polymorphisms with the same direction of effect on AUD and SCZ, 8 with robust effects in opposite directions, and 98 with disorder-specific effects. We also found evidence for 12 genes whose pleiotropic associations with AUD and SCZ are consistent with mediation via gene expression in the prefrontal cortex. The genetic covariance between AUD and SCZ was concentrated in genomic regions functional in brain tissues (p = 0.001).
ConclusionsOur findings provide further evidence that SCZ shares meaningful genetic overlap with AUD.

ABSTRACT IMPACT: We compare the cost-effectiveness of treatments for early prostate cancer, and propose how to maximize the value of care within an increasingly cost-constrained healthcare climate. OBJECTIVES/GOALS: Each year 192,000 men in the United States are diagnosed with prostate cancer. With various treatment options available, there is a growing role for cost-effectiveness analyses which may help maximize the value of care to the patient. In this review we compare the cost-effectiveness of primary treatments for clinically localized prostate cancer. METHODS/STUDY POPULATION: In this systematic review we aim to compare the cost-effectiveness or cost-utility of primary treatment strategies for clinically localized prostate cancer. This review, which adheres to 2009 PRISMA guidelines, included studies of men with clinically localized prostate cancer comparing at least two treatment strategies using the incremental cost-effectiveness ratio (ICER). We included analyses only of the United States healthcare system with at least 10 years of follow-up. These studies were published from 2006 to 2019 and generally included men with low or low to intermediate risk prostate cancer. Most studies reported outcomes for men age 65-70. All studies were prospective simulated trials and used a Markov model to simulate patient outcomes. RESULTS/ANTICIPATED RESULTS: Ten articles were included in the analysis. All studies used a Markov model to simulate a randomized trial. Six studies primarily compared radiation modalities, and four compared observation with immediate treatment. There was substantial heterogeneity in treatment protocols and the patients being simulated. Sensitivity analyses showed these models to be influenced by utility values and length of follow-up. A meta-analysis was not possible as no studies reported the variance of the primary outcome. Heterogeneity in study design limited comparisons of treatments across studies. However, these models were sensitive to patient-specific clinical factors, including life expectancy and the utility during and after each treatment. DISCUSSION/SIGNIFICANCE OF FINDINGS: These studies indicate collectively that the cost-effectiveness of prostate cancer treatment for similarly staged men may be heavily impacted by comorbidities and personal preferences. As the US moves towards value-based care, patient preferences may continue to drive the preferred treatment for newly diagnosed prostate cancer.

Bennett Island stands alone in a remote part of the Arctic and information on its geology is essential to ascertain relations with other terranes in order to restore the early Palaeozoic Arctic palaeogeography. Lower Palaeozoic sedimentary rocks throughout the island were studied thoroughly for the first time. The Ordovician section (> 1.1 km thick) comprises three units: Tremadocian, lowest Floian black shale (130–140 m); Floian, lower Dapingian carbonate turbidite (> 250 m); and Dapingian, lower Darriwilian siliciclastic turbidite (> 730 m). Ordovician deposits conformably overlie Cambrian rocks deposited within the Siberian shelf, as shown earlier. Most of the Ordovician succession was formed in a deep trough that received carbonate debris from a nearby carbonate platform and silicate material from a distant landmass located to the NE (present coordinates). The Bennett Island Ordovician rocks have much in common with those of both the Central and Northern Taimyr belts. It could be tentatively suggested that both belts merged at their eastern continuation in the vicinity of De Long Islands. The whole system probably extends further eastwards. The Ordovician facies patterns and faunal assemblages in the New Siberian Islands are notably similar to those of northwestern Alaska, where the same lateral transition from turbidites to shelf limestones was reported.
Maternal neglect and the serotonin system are associated with daytime sleep in infant rhesus monkeys

Environmental and biological factors contribute to sleep development during infancy. Parenting plays a particularly important role in modulating infant sleep, potentially via the serotonin system, which is itself involved in regulating infant sleep. We hypothesized that maternal neglect and serotonin system dysregulation would be associated with daytime sleep in infant rhesus monkeys. Subjects were nursery-reared infant rhesus macaques (n = 287). During the first month of life, daytime sleep-wake states were rated bihourly (0800–2100). Infants were considered neglected (n = 16) if before nursery-rearing, their mother repeatedly failed to retrieve them. Serotonin transporter genotype and concentrations of cerebrospinal fluid 5-hydroxyindoleacetic acid (5-HIAA) were used as markers of central serotonin system functioning. t tests showed that neglected infants were observed sleeping less frequently, weighed less, and had higher 5-HIAA than non-neglected nursery-reared infants. Regression revealed that serotonin transporter genotype moderated the relationship between 5-HIAA and daytime sleep: in subjects possessing the Ls genotype, there was a positive correlation between 5-HIAA and daytime sleep, whereas in subjects possessing the LL genotype there was no association. These results highlight the pivotal roles that parents and the serotonin system play in sleep development. Daytime sleep alterations observed in neglected infants may partially derive from serotonin system dysregulation.
Belomarinaite KNa(SO4): A new sulfate from 2012–2013 Tolbachik Fissure eruption, Kamchatka Peninsula, Russia

Belomarinaite, ideally KNaSO4, is a new sulfate mineral discovered in the Toludskoe lava field, formed during the 2012–2013 Tolbachik Fissure eruption. The mineral occurs as arborescent aggregates of tabular crystals (1 mm × 0.3 mm × 0.1 mm) comprising hematite impurities. The average size of the aggregates is 0.5–0.7 mm. The empirical formula is (K0.95Na0.92Cu0.04)Σ1.91S1.01O4. The crystal structure of belomarinaite was determined using single-crystal X-ray diffraction data; the space group is P3m1, a = 5.6072(3), c = 7.1781(4) Å, V = 195.45(2) Å3, Z = 2 and R1 = 2.6%. In the crystal structure of belomarinaite, there are six cation sites: the [4]S1 and [4]S2 sites are occupied by S, the [6]Na and [12]K sites are occupied by Na and K, respectively, giving Na0.5K0.5 apfu and the [10]M1 and [10]M2 sites are occupied by Na0.78K0.22 and K0.78Na0.22 apfu, respectively. The crystal structure is a framework of SO4 tetrahedra, Na octahedra and K, M1 and M2 polyhedra. Belomarinaite is isostructural with the synthetic compound KNaSO4. In belomarinaite, Na and K are disordered over M1 and M2 sites; in its synthetic analogue, Na and K are ordered over M1 and M2 sites, respectively. The Mohs’ hardness is 2–3. The mineral is uniaxial (+), with ω = 1.485(3) and ε = 1.488(3) (λ = 589 nm). The strongest lines of the powder X-ray diffraction pattern [d, Å (I, %) (hkl)] are: 4.022(31)(101); 3.591(26)(002); 2.884(74)(102); 2.800(100)(110); 2.391(16)(003); 2.296(8)201; 2.008(38)(022); and 1.634(10)(212). The mineral was named in honour of Russian volcanologist Marina Gennadievna Belousova (b. 1960) for her significant contributions to the monitoring of the Tolbachik Fissure eruption.

A number of laser facilities coming online all over the world promise the capability of high-power laser experiments with shot repetition rates between 1 and 10 Hz. Target availability and technical issues related to the interaction environment could become a bottleneck for the exploitation of such facilities. In this paper, we report on target needs for three different classes of experiments: dynamic compression physics, electron transport and isochoric heating, and laser-driven particle and radiation sources. We also review some of the most challenging issues in target fabrication and high repetition rate operation. Finally, we discuss current target supply strategies and future perspectives to establish a sustainable target provision infrastructure for advanced laser facilities.
An audit of Cryptosporidium and Giardia detection in Scottish National Health Service Diagnostic Microbiology Laboratories

Giardia duodenalis and Cryptosporidium species are protozoan parasites capable of causing gastrointestinal disease in humans and animals through the ingestion of infective faeces. Whereas Cryptosporidium species can be acquired locally or through foreign travel, there is the mis-conception that giardiasis is considered to be largely travel-associated, which results in differences in laboratory testing algorithms. In order to determine the level of variation in testing criteria and detection methods between diagnostic laboratories for both pathogens across Scotland, an audit was performed. Twenty Scottish diagnostic microbiology laboratories were invited to participate with questions on sample acceptance criteria, testing methods, testing rates and future plans for pathogen detection. Reponses were received from 19 of the 20 laboratories representing each of the 14 territorial Health Boards. Detection methods varied between laboratories with the majority performing microscopy, one using a lateral flow immunochromatographic antigen assay, another using a manually washed plate-based enzyme immunoassay (EIA) and one laboratory trialling a plate-based EIA automated with an EIA plate washer. Whereas all laboratories except one screened every stool for Cryptosporidium species, an important finding was that significant variation in the testing algorithm for detecting Giardia was noted with only four laboratories testing all diagnostic stools. The most common criteria were ‘travel history’ (11 laboratories) and/or ‘when requested’ (14 laboratories). Despite only a small proportion of stools being examined in 15 laboratories for Giardia (2%–18% of the total number of stools submitted), of interest is the finding that a higher positivity rate was observed for Giardia than Cryptosporidium in 10 of these 15 laboratories. These findings highlight that the underreporting of Giardia in Scotland is likely based on current selection and testing algorithms.

Paleoproductivity records from Lake Baikal indicate that the last interglaciation in southeastern Siberia lasted ca. 12,000 yr from 127,000 to 115,000 yr B.P., was punctuated by a climatic oscillation about 120,000 yr ago, and ended abruptly with a rapid onset of regional glaciation during marine isotope substage (MIS) 5d. The Siberian equivalent of the MIS 5e–5d transition appears to lead the post-Eemian cold pulses in Europe and in the central North Atlantic by 5000 to 8000 yr. The Baikal record also registers a pronounced cold event during early MIS 5c, which correlates with the Montaigu event in pollen records from France. Second-generation Glyphosate-Resistant Cotton Tolerance to Combinations of Glyphosate with Insecticides and Mepiquat Chloride

Field trials were conducted in 2005 and 2006 to evaluate application of glyphosate alone or plus the plant growth regulator mepiquat chloride with 20 different insecticides to second-generation glyphosate-resistant cotton at the pinhead square or first bloom growth stages. At 7 DAT, averaged across cotton growth stages and herbicide treatments, combination with insecticides profenofos and methomyl resulted in 5 and 9% plant injury, respectively, and were the only insecticide combinations that resulted in injury greater than glyphosate or glyphosate plus mepiquat chloride applied alone. By 14 DAT, cotton injury was less than 2% for all treatments. Averaged across cotton growth stages and insecticides, addition of mepiquat chloride to glyphosate resulted in a 4 and 6 cm height reduction at 7 and 28 DAT, respectively. Seed cotton yield and percent first harvest were similar for all treatments, indicating that cotton injury and height reductions observed after application did not result in yield reductions or maturity delays. Glyphosate combined with insecticides and mepiquat chloride, in accordance with herbicide labeling for second-generation glyphosate-resistant cotton, offers producers the ability to integrate pest and crop management strategies and reduce application costs with minimal effect on the crop.

Field trials were conducted during 2006 and 2007 and a container study was performed twice in 2007 at the Dean Lee Research and Extension Center in Alexandria, LA to evaluate the interaction of glyphosate and zinc coapplied to selected weeds. Across all experiments, no differences in either visible weed control or weed fresh weight were detected among glyphosate formulations. In the field studies, weed control was greatest when glyphosate was applied alone, in which case control of barnyardgrass, browntop millet, and Palmer amaranth ranged between 93 and 95%. When glyphosate was coapplied with formulations of zinc, control of the aforementioned weed species was reduced to 39, 39, and 45%, respectively. Visual estimates of weed control in the container studies showed glyphosate performance to be the highest (82 to 98%) in the absence of zinc for control of barnyardgrass, browntop millet, johnsongrass, ivyleaf morningglory, and redroot pigweed. Across all weed species, control was reduced 43 to 59% when zinc was coapplied with glyphosate. Similar results were noted in reduction of weed fresh weights. Results indicate that glyphosate-based weed control is reduced when coapplied with the zinc products at their current use rates. Producers should be aware of this antagonism and these coapplications should not be recommended.

Approximately half of the variation in wellbeing measures overlaps with variation in personality traits. Studies of non-human primate pedigrees and human twins suggest that this is due to common genetic influences. We tested whether personality polygenic scores for the NEO Five-Factor Inventory (NEO-FFI) domains and for item response theory (IRT) derived extraversion and neuroticism scores predict variance in wellbeing measures. Polygenic scores were based on published genome-wide association (GWA) results in over 17,000 individuals for the NEO-FFI and in over 63,000 for the IRT extraversion and neuroticism traits. The NEO-FFI polygenic scores were used to predict life satisfaction in 7 cohorts, positive affect in 12 cohorts, and general wellbeing in 1 cohort (maximal N = 46,508). Meta-analysis of these results showed no significant association between NEO-FFI personality polygenic scores and the wellbeing measures. IRT extraversion and neuroticism polygenic scores were used to predict life satisfaction and positive affect in almost 37,000 individuals from UK Biobank. Significant positive associations (effect sizes <0.05%) were observed between the extraversion polygenic score and wellbeing measures, and a negative association was observed between the polygenic neuroticism score and life satisfaction. Furthermore, using GWA data, genetic correlations of -0.49 and -0.55 were estimated between neuroticism with life satisfaction and positive affect, respectively. The moderate genetic correlation between neuroticism and wellbeing is in line with twin research showing that genetic influences on wellbeing are also shared with other independent personality domains.

Introduction
Hypoplastic left heart syndrome with an intact atrial septum is a poor predictor of outcomes. Prenatal assessment of pulmonary venous Doppler and emergent postnatal cardiac intervention may be associated with better outcomes.
Materials and methodsA retrospective review of all hypoplastic left heart syndrome patients in two centres over a 5-year period was performed. Group 1 included patients with adequate inter-atrial communication. Group 2 included patients with prenatal diagnosis with an intact atrial septum who had immediate transcatheter intervention. Group 3 included patients with intact atrial septum who were not prenatally diagnosed and underwent either delayed intervention or no intervention before stage 1 palliation. Primary outcome was survival up to stage 2 palliation.
ResultsThe incidence of hypoplastic left heart syndrome with a restrictive atrial communication was 11.2% (n=19 of 170). Overall survival to stage 2 or heart transplantation was 85% and 67% for Groups 1 and 2, respectively (n=129/151, n=8/12; p=0.03), and 0% (n=0/7) for Group 3. Survival benefits were observed between Groups 2 and 3 (p<0.001). Foetal pulmonary vein Doppler reverse/forward velocity time integral ratio of ⩾18% (sensitivity, 0.99, 95% CI, 0.58–1; specificity, 0.99, 95% CI, 0.96–1) was predictive of the need for emergent left atrial decompression.
ConclusionUsing a multidisciplinary approach and foetal pulmonary vein Doppler, time-saving measures can be instituted by delivering prenatally diagnosed neonates with hypoplastic left heart syndrome with intact atrial septum close to the cardiac catheterisation suite where left atrial decompression can be performed quickly and safely that may improve survival.
